- Leads the development and review of measurable outcomes against which the community design and performance may be judged and identify gaps that will need to be addressed to enhance service delivery. - Leads in monitoring and reporting on community projects using appropriate tracking and feedback systems. - Directs and participates in surveys, assessments and evaluation of activities to determine the outcomes and health promotional needs within the target groups. - Leads in the provision of functional expertise in matters relating to community data integrity, solving systems-related problems and defining requirements for necessary changes. 5.
